One switch stood between North Carolina and a bomb 260 times stronger than Hiroshima
On January 24, 1961, a B-52 bomber broke apart in midair over Goldsboro, North Carolina, dropping two Mark 39 hydrogen bombs on the countryside below. A Sandia National Laboratories report, kept classified for decades and released in stages beginning in 2013 following a FOIA request from journalist Eric Schlosser, confirms that one of the two bombs went through nearly its entire arming sequence during the fall. By the time it hit the ground, only one safety switch, out of four, had not activated. Had that switch also failed, the device would very likely have detonated, at roughly 260 times the explosive force of the bomb dropped on Hiroshima.

The force of the crash itself, not any human error or malfunction unrelated to the accident, triggered the mechanical fuzing sequence, the specific chain of steps required to arm a nuclear weapon, on both devices. One bomb landed in a field and was recovered relatively intact. The other completed every step of that arming sequence except one, a pre-arming switch operated by a 28-volt signal, before crashing into the ground.
A later declassified Sandia review found that the exact same type of switch involved at Goldsboro had failed inadvertently in roughly 30 other incidents across the broader nuclear stockpile. In other words, the specific safety mechanism credited with preventing detonation at Goldsboro was not a uniquely reliable design, it was a component with a real, documented history of failing under other circumstances. A separate 1986 Sandia study went further, concluding the United States could not claim, in an absolute sense, that it took every action necessary to ensure the safety of its fully assembled, combat-ready nuclear weapons.
This establishes that a real nuclear weapon, involved in a real 1961 accident on American soil, came far closer to detonating than official accounts acknowledged for decades, confirmed directly by Sandia's own internal safety engineers in a real, declassified technical report, not by outside speculation. This is confirmed by multiple staged FOIA releases from 2013 through 2022.
What this does not establish is that detonation was a certainty had the final switch also failed; Sandia's own engineers describe the outcome as credible and possible under the right conditions, a serious technical judgment, but a probabilistic one rather than an absolute guarantee either way.
